盆底肌生物反馈电刺激对脊髓损伤后神经源性膀胱功能恢复的临床研究

摘要
目的:观察盆底肌生物反馈电刺激治疗脊髓损伤后神经源性膀胱的疗效。

方法:将36例神经源性膀胱的脊髓损伤患者分为两组:治疗组18例患者每日给予盆底肌生物反馈电刺激和标准护理干预治疗;对照组18例患者每日仅给予标准护理干预治疗。两组分别进行8周的治疗。治疗前后患者采用排尿日志、尿动力学指标、生存质量评分和国际下尿路症状评分进行疗效评价。

结果:两组患者各项指标在治疗后均有不同程度的恢复和提高(P<0. 05),但膀胱压力无明显变化(P>0. 05)。治疗组患者在排尿次数、日均排尿量、膀胱容量、最大尿流率、生存质量评分及国际下尿路症状评分上的改善更优于对照组(P<0. 05) 。

结论:盆底肌生物反馈电刺激联合护理干预能显著改善脊髓损伤患者神经源性膀胱的功能,且对患者的生存质量有较为明显的提升,值得临床推广。

Effects of pelvic biofeedback electrical stimulation on neurogenic bladder in patients with spinal cord injury

Abstract
Objective:
To evaluate the clinical effects of pelvic biofeedback electrical stimulation on neurogenic bladder in patients with spinal cord injury(SCI).

Methods: Thirty-six SCI patients with neurogenic bladder were randomly divided into 2 groups, a treatment group (18 patients underwent pelvic biofeedback electrical stimulation and standard bladder nursing intervention everyday for 8 weeks) and a control group (the other 18 patients underwent standard bladder nursing intervention). The voiding dairy, urine dynamics testing, quality of life scores and the international lower urinary tract symptoms (LUTS) scores were recorded during the treatment.

Result: The patients in both groups got a significant recovery of their bladder function(P<0.05), however, the bladder pressure had no significant difference before and after treatment(P>0.05). The patients in the treatment group had a better improvement in the mean urinary voiding volume(P<0.05), bladder capacity(P<0.05), maximum urinary flow rate(P<0.05), quality of life scores(P<0.05) and LUST scores(P<0.05) and a less mean frequency of urinary voiding(P<0.05) as compared to those in the control group.

Conclusion: Pelvic biofeedback electrical stimulation improve with nursing intervention can significantly umprove the neurogenic bladder function in SCI patients, and consequently improve the patient’life quality.
